A 54 year old patient came to casuality with incisional herina

A 54 year old female patient, resident of adauidulpally, Nalgonda district tobacco worker  by occupation came to our hospital with chief complaints of -

1. Inicisional hernia
2. Swelling in right side of abdomin 
3. Hernioplasty

Date of admission :- 15/03/2022

History of present illness:-

Patient was apparently asymptomatic 5 days back , then she noticed a swelling in the hypogastrum extending to right side iliac fossa(10×15 cms) which was reducible on lying on and protruding out on raised intra
abdominal pressure 
Tendreness present 
Cough present

No H/o of vomitings, loose stools,No H/o of constipation, No H/o chronic cough present, No H/o of trauma

History of past illness:-

 H/o of hysteroctomy then due perforation of uterus due to pyometry two years ago 
H/o of hypothyroid (was on medication)
Not a known case of diabetes, hypertension, tb, epilepsy, asthma 

Personal history:-

Patient has normal eating habit with mixed diet.
Occasional  alchohol consumption and no smoking.


Daily routine:-

Patient used to wake up at 7.00AM , takes breakfast at 9.00AM .Then she goes to her  work and takes rice in between 4.00PM-5.00PM and dinner at 8.00PM and goes to bed by 9.00PM.


Family history:-

No significant family history.

General examination:-

Patient is conscious, coherent, cooperative and well oriented to time, place and person.

Pallor present
No icterus
No clubbing
No lymphadenopathy

Vitals:-

Temperature-98 F
Pulse rate- 86 bpm
RR-16 /min
BP-110/70 mmhg
SPO2-98% at room air
RBS-108 mg/dl

Provisional diagnosis:-

  Incisional hernia 

Treatment-

T. PAN 40 mg  PO/OD

T. MVT  PO/OD 

T. OROFERXT  PO/BD 

T. VIT-C 500Mg PO/OD

SYP ACSORYC 2 Spoons TID

Surgery Suggested
